Once everyone’s happy, we’ll work with you and the participant to put the right support in place.
Nextstep Community Care collects the information in this form, including health and disability information, to assess and respond to this referral. Where you are referring another person, you confirm you have their consent to share their information with us. We handle this information confidentially and in line with the Privacy Act 1988 and our Privacy Policy. We will only use it to assess, provide or coordinate supports.
